What is the HealthyCare Card? 

The HealthyCare Card is a new program which provides discounts to care for those who require financial assistance with their medical care.

How Does a Person Find Out About ther HealthyCare Card? 

  • If a person is seen at one of the health care providers listed above and indicates financial hardship.
  • If a person is receiving assistance from on of the agencies listed above and has health care issues which need to be addressed.
  • Through other organizations and agencies in the York community.
  • Friends, neighbors or family.

How Does the Process Work? 

  • Person completes the application.
  • The form is evaluated using the income, expense, and asset information provided by the applicant.
  • If applicant is eligible for health insurance, you will need to pursue these options before submitting this application. 
  • If the person is eligible for Healthy Community Network, they will be given a plastic card to present when they need care at WellSpan Health, Memorial, Family First Health or Hanover Hospital.
  • If a person doesn’t have a family doctor, Healthy Community Network will direct them to a location where they can see a family doctor.
  • People are encouraged to use their family doctor for the purpose of continuing care, though no one will be refused care if they show up at another provider.
  • Every year or six months a person must re-apply for the HealthyCare Card. The expiration date is on the card.
  • Those who qualify for Healthy Community Network also qualify to receive their medications from the medication assistance program.

Does HealthyCare Card Cost Anything? 

  • There is no fee to belong to Healthy Community Network at this time.
  • The discount on care will depend on the doctor and the person’s financial status.
  • People will receive discounts from the doctors participating within the Healthy Community Network.  Doctors outside of Healthy Community Network may or may not discount care.  Patients are encouraged to talk with their doctors about fees they may be charged.
  • A $15 co-pay will be charged for office visits.
  • A $20 co-pay will be charged at ReadyCare.
  • A $30 co-pay will be charge at Emergency Room.
  • Family First Health uses a different method of discounting services.  Please contact them directly for information at 845-8617.

Why do we require copies of household bills?

Effective January 1, 2010 we require Medicare eligible applicants to complete this section of the application.  Review of household bills will help us to guide the applicant to other possible assistance.  For example a copy of one month of each utility bill, statement for mortgage payment or a receipt of rent payment is needed.

What is proof of income?

All income types must be submitted by applicant and his or her household members.

Examples of Proof of Income:

  • Pay stubs - most current for one (1) month with consecutive dates.
  • Federal 1040 Tax Return – most recent year.
  • Statement from Medicare/SSI.
  • Letter from Employer.
  • Self employed must submit a 1040 & Schedule C and/or Schedule E.

Examples of Non-Wage Income:

  • Social Security Statements
  • Disability Income
  • Unemployment Income
  • Workman's Compensation Income
  • Pension
  • Investment Income (including Rental Property)
  • Child Support/Alimony
  • Public Assistance (Cash or Food Stamps)
  • If separated – must have spouse’s income or legal documentation of separation

What other documents are required?

  • Provide a copy of Photo ID for each person applying
  • Copy of all insurance cards
  • Bank statements for all accounts held and all the pages of each statement
  • For self-emplolyed: copies of 3 months of all household & business accounts (include all pages)

How do I know if I would qualify for HealthyCare Card Program?

Eligibility is based on the financial assistance policies of each of the local health systems. We follow Federal Income guidelines.

What Else Does the HealthyCare Card Program Provide? 

  • Access to doctor visits, emergency visits, labs, x-rays and other testing.
  • Healthy Community Network patients who have certain chronic conditions are enrolled in “HealthCompanion” which help people understand and care for their disease more effectively.
  • HealthCompanion provides focused oversight of care for patients with specific diseases.
  • Access to specialty services through York Hospital Specialty Clinic and other community providers.
  • Medications will be provided through the Healthy Community Pharmacy and Hanover Apothecary.
  • For brand name drugs not available at the pharmacy, staff will assist with completing pharmacy assistance forms for $5 a form.
