Healthy Community Network, formerly known as York Network (HYN), was founded by WellSpan Health in 2003 in an effort to better manage patients without insurance who use WellSpan services.  The goal was to provide the uninsured a way to receive care in a coordinated and managed fashion, while reducing the costs associated with this care.  As the number of patients in the programs increased, a decision was made by WellSpan to apply for a Federal grant given to communities to specifically improve coordination of care to the uninsured.  It was at this point that other service providers of the York area were invited to join in this effort.  A three-year grant was awarded to WellSpan Health who will in turn share the Federal funding with the HYN providers.
